Likewise, we do usually a battery of blood tests.
I usually test vitamin levels, B12 and folic acid. I test for thyroid dysfunction. I examine all medications that a person is receiving. Elavil, amongst all of the anti-depressants, has a very powerful anti-cholinergic side effect. Then, there could be other health problems. People who have severe sleep apnea, where their oxygen levels drop at night precipitously down, can wake up in the morning groggy and confused and be falling asleep all the time.
We can fix that with this little device that helps you keep your oxygen level up at night called CPAP. You can look at all different kinds of staging systems. We use them in research studies. The ones I found in talking to many caregivers, they like to keep things simple, and there are early, middle and late stages.
Typically, individuals do not have a lot of psychiatric problems. There are the intellectual and then there are the psychiatric and behavioral. In the middle stages, however, the patients develop a broader range of symptoms. The medical Giana Brotherz / Dementia (7) & Nme Click - WГјstensturm (2008 Rmx) / Always On My Mind (Vinyl) is amnesia, aphasia, apraxia, agnosia. In the middle stages, the patients classically have rather profound mixtures of those symptoms, and oftentimes they also have some psychiatric or behavioral symptoms.
They may become irritable. In some instances, they have false beliefs. Sometimes people even come to the conclusion that their spouse is having a romance with the person next door. There are all kinds of symptoms that can come on psychiatrically in the middle stages. Giana Brotherz / Dementia (7) & Nme Click - WГјstensturm (2008 Rmx) / Always On My Mind (Vinyl) the later stages, the losses become more profound.
Patients then begin to forget how to walk, they forget how to chew and how to swallow. They forget how to talk. They forget the faces even of their loved ones, their caregivers.
Oftentimes, they take to their bed. The way your brain works is you have this part of your brain that stores information for short term. So they have short term recall problems. They tell the same story three times in the same evening. They ask the same question over and over again. They forget where their wallet is repeatedly. They forget really significant, important things that they were supposed to do.
Then the second symptom they get early on is they start stumbling over their words. Everybody has memory problems. We divide memory troubles into sort of three or four categories as you get older. In mild cognitive impairment, they have a really significant memory problem to the point where they get lost going to the store, or they get to the store and forget why they were there.
However, they do not have any of language or motor skill problems. They forget really important appointments. When you test these people with standardized paper and pencil testing, oftentimes everything else but memory looks pretty good. They will score into the abnormal range on the memory testing. If you look at older individuals with mild cognitive impairment, a substantial number of them over five years will move on to a more serious disorder called dementia.
That means that in addition to having the memory problems, they also have other intellectual deficits. How many patients who have mild cognitive impairment move on to dementia? It depends on how old they are. Younger people who have these memory problems seem to progress less often than older people. However, between 40 and 60 percent of people with mild cognitive impairment are going to move on to develop dementia. When someone goes to the doctor, what questions should they be sure to ask their doctor?
These symptoms include the individual becoming more forgetful, having trouble concentrating and having difficulty Giana Brotherz / Dementia (7) & Nme Click - WГјstensturm (2008 Rmx) / Always On My Mind (Vinyl) familiar tasks. The importance of the diagnosis is simple because there are many contributing factors to memory loss. There can be a vitamin B deficiency.
There can be a reaction to different treatments or prescriptions that a person might be on that might be bringing about some of the things that I just mentioned about, forgetfulness and trouble concentrating.
Your doctor can help. You can ask about the diagnosis. You can ask if there tests that you should taking and what some of the treatment options are. Across the country, there are many more competent doctors making proper diagnosis and treatment in this regard. So they work in some people. What are the different kinds of the treatments? The first line of treatment includes medicines that enhance the amount of a chemical transmitter in the brain called acetylcholine.
Acetylcholine is this neurotransmitter. Usually, these drugs work better in the earlier, initial stages than in the later stages, although recent studies indicate that Aricept is beneficial in the later stages as well. How do they work?
Well, they slow the progress of the disease. How do you measure that slowing? We know that the studies indicate that you can keep a person out of a nursing home by about two years if you treat them with one of these medications.
There is a second kind of medicine called Namenda memantinewhich is typically used in the middle to later stages, Giana Brotherz / Dementia (7) & Nme Click - WГјstensturm (2008 Rmx) / Always On My Mind (Vinyl).
It blocks damage in the brain. It works in a different way than the cholinesterase inhibitors. These medicines are effective at slowing the progress. The studies show that if I add Namenda to a drug like Aricept or Razadyne, patients get a bump in the effect.
Oftentimes, patients develop depression in the course of dementia. If you treat them with an anti-depressant, just like you would other folks who have depression, it seems to really help a great deal. There are medications that are available, and there are medicines just over the horizon that are going to come on the market probably in the next two to five years, which I think are going to work even better than these medicines.
Powers, back to the cholinesterase inhibitors and the memantine, are there significant side effects with those drugs? Longo on Dementia Play Video Clinical Trials Clinical trials are research studies Giana Brotherz / Dementia (7) & Nme Click - WГјstensturm (2008 Rmx) / Always On My Mind (Vinyl) evaluate a new medical approach, device, drug, or other treatment.
